If you are unsure of who to speak with or if you need to reach a specific staff member, please call us at (480) 397-7577, 24 hours a day, and we will help you locate someone who can help. Our Administrative Office is open M-F 9am-4pm, closed an hour for lunch and on Federal Holidays.
Our Administrative Office is located in the Superstition Springs Corporate Center at the southwest corner of Power Rd. and Southern Ave in Mesa. We are on the first floor in Suite 150 to the left.
The Joint Commission conducts accreditation surveys of Allegiant Hospice on an unannounced basis. The purpose of a survey is to evaluate the organization’s compliance with nationally established Joint Commission standards. Joint Commission standards deal with organization quality, safety-of-care issues, and the safety of the environment in which care is provided. Anyone believing that he or she has pertinent and valid information about such matters is encouraged to contact the organization’s management. If the concerns in question cannot be resolved at this level, please contact a Joint Commission field representative by telephone at 1-800-994-6610. Information presented will be carefully evaluated for relevance to the accreditation process.
